REFERRAL FORM
This form refers a current UNL student, faculty, or staff member to an Advocate at the Center for Advocacy, Response & Education (CARE). If you are not the victim/survivor being referred, you are urged to obtain consent from the victim/survivor to share the information with CARE prior to submitting this form. Working with an Advocate is a choice that must be made by the victim/survivor.

ABOUT CARE
CARE is a confidential, supportive resource. We provide advocacy and support for students, faculty and staff who have experienced sexual assault, domestic/dating violence, stalking, or harassment. Advocates are here to help navigate campus and community resources.
- With a victim advocate, one can tell their story confidentially.
- Victims/Survivors will be supported in their choices and decisions.
- A victim/survivor can choose to report to police or IEC/Title IX, or not at all. The choice is 100% up to the victim/survivor.


If a Victim/Survivor needs immediate assistance outside our regular hours (M-F, 8 a.m. - 5 p.m.), they may speak with an on-call counselor through UNL Counseling and Psychological Services (CAPS) at (402.472.7450) or off-campus partners Voices of Hope at (402.475.7273).
