Common Measures Reporting Template

Create custom impact reports using the 4-H Common Measures Reporting Template! The Reporting Template easily allows you to convert a Qualtrics data file into a customizable impact report complete with an introduction, body, and conclusion. Developed in Microsoft Word, you will have the ability to add additional information, data, and pictures after the report has been generated! Instructions

  1. Export your survey data from Qualtrics as a Comma Separated Values (CSV) file. Ensure you select the “Use Choice Text” option when downloading your data (If Numeric Values are selected your impact report will reflect numbers instead of phrases for question answer choices). For instructions on how to download your data, please refer to the Qualtrics Exporting Response Data support page.

  2. Complete the fields included below
    1. Program Name: What 4-H program(s) does this data represent?
    2. Program Time Period: When, or during what time frame, was this data collected?
    3. Upload Qualtrics Data CSV: Locate and select your CSV file of data (only one file can be uploaded).
    4. Upload Cover Image (optional): If applicable, include an image from your program. A stock photo will default if no custom photo is included.
    5. Upload Logo (optional):Consider including your universities logo. The 4-H Clover will default if no custom logo is included.
    6. Remove Long Question Responses: It’s recommended to check this box as the Reporting Template is not designed to analyze or include qualitative data. Qualitative data should be analyzed externally and manually added after the report is generated.

  3. Review the generated impact report and continue customizing!
